Requesting Vital Records
You must fill out a request form, which you may either obtain in person at the Health Center, or may download and print from here. Each copy has a cost of $15.00 each for Birth Certificates and $13.00 for the first Death Certificate, and $10.00 for each additional copies purchased on the same day, which must accompany the application. Please make check or money order payable to:Pettis County Health CenterDo not send cash by mail!
Please be as complete as possible in your application. Records are filed by year of the event and then alphabetically by the name of the person at the time of the event. Therefore, at least the month and year of birth or death, and the first and last name of the registrant must be given before a search can be made. Fill out your personal information completely, including a minimum of one parents name for death certificates and both parents names for birth certificates, if listed. Also, include a day time phone number in which you may be reached should we need more information. If we cannot contact you, we may not be able to complete your request.
